Adminsitrative Fee for Paperwork (L1)

This fee applies to the completion of medical forms, letters, reports, or other documentation requested by the patient or a third party that is not covered by OHIP. Fees are based on the time and complexity required to complete the paperwork and are consistent with Ontario Medical Association guidance for uninsured services.
